Background Investigation


Instructions to Candidates

CourtsNew York State Court Officer Candidates who successfully pass the pre-screen medical examination must follow the instructions below and bring the following documentation when returning for the full medical examination.

1. Download and Complete:

  1. PERSONAL HISTORY QUESTIONNAIRE BOOKLET (UF-1)
    Complete the booklet in its entirety. EVERY QUESTION MUST BE ANSWERED (you may indicate, “does not apply,” only when applicable). Answer every question with truthfulness and without evasion.
  2. NOTICE OF ARREST (UF-1a)
  3. CHANGE OF INFORMATION FORM (UF-11a)
  4. AUTHORIZATION FORM (UF-3)
  5. NYC DEPT OF SOCIAL SERVICES form
  6. NYS DEPT OF LABOR form

2. You Must Obtain a Detailed Income Earnings Printout Form (SSA-7050) from the Social Security Administration and Submit at Full Medical Examination.

  1. Printout must cover the period from the year you left High School to present.
  2. You must obtain this form by visiting a SOCIAL SECURITY ADMINISTRATION office. Form may be obtained on the same day if requested in-person. Requests by mail may take up to 6 to 8 weeks delaying your investigation.

3. Copies of Arrest Information Must Be Provided at the Full Medical Exam

  1. Candidates must provide certificates of disposition (copies) for every and all arrests and/or summonses.

YOU MUST HAVE THE ABOVE DOCUMENTS WHEN REPORTING FOR YOUR FULL MEDICAL EXAM
