To receive credit for jury service, complete the fields below.  The fields designated with an * are required fields and must be completed or the notification will not be accepted by the Court.

If you decide not to utilize the Online Notification System, in order to receive credit for jury service, you must mail, to the Court, the juror information form which you received with your summons or click here for the website printable form.    If, however, you use the Online Notification System, you will not need to mail the juror information form to the Court.

When you have completed all necessary fields, click on the SEND TO COURT button to transmit the information to the Court. You will receive an acknowledgement page which reiterates the information you provided to the Court. You may wish to either print this response using your browser print button or save to your hard drive as proof of information you provided to the Court.
