Need an ADA Accommodation?

Note: this Form is not to be used to make blanket requests for remote appearances by phone or video, either in more than one case before the same judge, or in cases before more than one judge. Instead, please use other means to contact the judge or judges before whom the cases are pending.

Parking PARKING: No public parking on-site or adjacent to the facility. Public parking garages are nearby for a fee.

ADA ENTRANCE(S): Pearl Street (southside of the building). Enter on street-level via an ADA-accessible elevator that may be operated by uniformed personnel at all times during business hours.

Do we want to excise the reference to the van in the text underneath the photo OR add after “walkway, which may not always be parked there during business hours.”  Further, we should consider adding that “There is ADA signage both to the left and right of the door.”  (Description only reads “to the right” despite signs that are clearly pictured to the left and right.)

Description of photo: There is a long walkway that leads to a single door entrance. There is ADA signage both to the left and right of the door. There is a New York State Unified Court System van parked to the left on the walkway, which may not always be parked there during business hours.
