Filing a New Business Registration
To file for a new business registration, you will need to submit the appropriate business forms:
- To file as a single proprietor, the Blumberg form is X201.
- To file as a partnership, the Blumberg form is X74.
- To amend a business certificate, please file form T224.
The above forms may be purchased at any stationery store that carries legal forms. The County Clerk requires three notarized forms. The fee of $120 is payable to the Kings County Clerk via cash, certified check, or money order.
If filing by mail, please include three notarized copies of the appropriate forms with $120 payable to the Kings County Clerk by certified check or money order. Please include a stamped self-addressed envelope for your receipt. Mail your request to:

Filing Jury Demands
To file a jury demand, submit three copies to the County Clerk. The filing fee is $65. This fee is payable to the Kings County Clerk in cash, money order, certified check, or New York State attorney's check.

If filing in person, you must first have the forms approved by Motion Support in Room 227 before proceeding to Window #5 with your three approved copies.
Filing Notices of Appeal to Appellate Division, 2nd Department
To file a Notice of Appeal to the Appellate Division, 2nd Department, please submit the following documents in three collated sets, each consisting of:
- Notice of Appeal
- Radi (Blumberg Form A477)
- The copy of what you are appealing, and the original Affidavit of Service of the entire package on the other parties, back must contain the attorney's name, address, and telephone number. Note: no pre-argument statement is required.
The filing fee is $65. This fee is payable to the Kings County Clerk in cash, money order, certified check, or New York State attorney's check.

Filing Notes of Issue
For a Note of Issue in a Matrimonial Action
To file a note of issue in matrimonial action, submit three note of issue forms to the County Clerk. The filing fee is $125. This fee is payableto the Kings County Clerk via cash, money order, certified check, or New York State attorney's check.

For a Note of Issue in an Action Other than Matrimonial
For actions other than matrimonial, submit three copies to the County Clerk with a filing fee of $30, plus $65 if jury demanded. Fees are payable to the Kings County Clerk via cash, money order, certified check, or New York State attorney's check.

Filing a Religious Corporation
To file a religious corporation, submit an original and two copies with a legal back affixed to the original only. Please note: there are no printed forms for the filing of a religious corporation. The fee is $120, payable to the Kings County Clerk in cash, money order, or certified check. Include a stamped self-addressed return envelope for the return of your copies and receipt.

Filing an RJI
The fee for filing an RJI is $95.
This fee is payable to Kings County Clerk in either cash, money order, certified check or New York State Attorney's check. You must submit three RJI forms, filled out on both sides and signed. No affidavit of service is required. Please mail the required forms and fees and a self-addressed stamped envelope for your receipt to:

Filing Supreme Court Papers
All documents must contain the attorney's name, address and telephone, index numbers and comply with Part 130 of the Rules of the Chief Administrator. All third party actions must contain both the main caption and index number, as well as the third party caption and index number.
